There are a lot of reasons for why you might feel the way you do. Guilt, as you menitoned, realization of your own mortality, etc. Whatever the reasons are, your feelings are entirely justified. But they ARE complex ones and a counselor might be able to help you with specific things about the accident that are hard for you to forget.
In any case, you're not alone. And a lot of people can help you. What you can do to help yourself if try to figure out why it is that the incident makes you feel the way you do. Talk to someone a little older and a little wiser who can help you sort those things out.
To go through this kind of thing in high school is terrible, but it's something that does happen. And there are people who can help you.
I really hope your mind finds some peace about the accident. My prayers go out to you ...
